Complaint to WorkSafeBC Issue Resolution Office (Form 15B3)
To register a complaint with the WorkSafeBC Issue Resolution Office (formerly known as the Fair Practices Office), please:
- Complete this form and mail or fax it to us, or
- Use the online complaint portal instead
Before you begin, it may be helpful to review the Issue Resolution Office page to learn more about us, what we can investigate, and other ways you may submit your complaint. For example, we can only investigate issues that are current or have occurred within the last 12 months.
Before submitting a complaint to the WorkSafeBC Issue Resolution Office, you must raise the complaint with the appropriate manager in the department you are complaining about. If you are unsure which manager to contact, you can find out by calling the divisional call centre or department.
