WorkSafeBC does not currently have a formal agreement with prosthetists, however we may authorize prosthetic services performed by the following qualified providers:
Certified Prosthetist – C.P. (c) or
Certified Prosthetists and Orthotists – C.P.O. (c)
Providers certified by the Canadian Board for Certification of Prosthetists and Orthotists (CBCPO) are eligible to provide prosthetic services to injured workers.
Phone the WorkSafeBC Claims Call Centre to determine if a claim has been submitted and accepted.
It is essential that a WorkSafeBC officer pre-authorization is obtained in writing prior to providing any prosthetic devices and/or services by completing Form 83D19: Request for Authorization for Prosthetic Services.
Fax the completed authorization form to the WorkSafeBC officer for approval prior to providing the service:
Upon receipt of approval from the WorkSafeBC officer, please provide the injured worker with the appropriate devices and services, and complete Form 83D13: Invoice for Prosthetic Services.
When completing the invoice, please indicate the appropriate item code for the device supplied under the “Fee code” column.
For assistance in selecting the correct item code, please refer to the September 2010 Prosthetic Providers Information Bulletin.
