Apply for compensation and report a work-related injury or occupational disease using one of the options below. You’ll need details of the incident that caused your injury, your earnings information if you missed work, as well health care provider information if you sought medical treatment.

Option 1: Call Teleclaim

If you’ve missed work as a result of your injury, start a claim by calling Teleclaim at 1 888 WORKERS (1 888 967-5377), Monday to Friday from 8 a.m. to 6 p.m. The claims representative will also give you a user ID and password so you can go online later to check your claim details.

Option 2: Submit your form online

If you’ve missed work as a result of your injury, and if you have a user ID and password, you can submit your Form 6 online using the first button below. The advantage of having this type of online account is that you can then log on later to see more details about your claim, including correspondence, claim decisions, and payment information.

Option 3: Report injuries not resulting in loss of time from work

If you have not missed work as a result of your injury, all that is required is that you report the details about the incident to your employer as soon as possible.
